As an in-home, LPN nurse-assisted service, the Pinnacle Plus+ program helps families, friends, and caregivers by taking care of many of the most challenging tasks related to caring for a loved one.

The goal of this program is to help your loved one manage their medications from the comfort of their own home, improving their overall physical health and mental well-being.

How much does the Pinnacle Plus+ program cost?

The short answer is “it depends”. The cost to enrol in the Pinnacle Plus+ program is determined by a variety of factors, including which medications the patient takes, income levels, and insurance benefits. Each patient has unique factors that play into this, but we can say the majority of our patients are surprised at how little the program costs for the level of care they receive.

Are there specific eligibility criteria to join the Pinnacle Plus+ Program?

The person enrolled in the service must require regular daily medications to be eligible for Pinnacle Plus+.


How many visits or hours are provided under the Pinnacle Plus+ program?

Pinnacle Plus+ provides a daily visit for each patient, for as long as they remain within the program.


What credentials do Pinnacle Plus+ caregivers have?

Pharmacists prepare and educate on the medication, while LPNs administer the medications and carry out other aspects of the care plans. Many of our LPNs have additional training to expand their scope (for example, licensed footcare nurses), and our Pharmacist Lead is a Certified Diabetes Educator.


Do you always send the same nurse during Pinnacle Plus+ home visits?

Pinnacle Pharmacy is a small, dedicated team focused on personal customer care. Our patients always see a member of our team who they know during Pinnacle Plus+ home visits, which facilitates a connection between patient and nurse. When our team expands, we send our new nurses out with one of our experienced nurses to be introduced to the patients beforehand, so that they are already familiarized with our new team members before they do their first solo patient care visit.


What measures are in place to ensure consistent quality of care across different caregivers?

The advantage of a close-knit team is that we are in constant communication with one another. A detailed communication log is written daily with any important details outlined for the nurse on duty the next day. We also have electronic charting so it is easy for team members to check vitals from previous days, report on any new concerns, or monitor previously noted things.


Can family members help with care planning and decision making?

Absolutely! Family members often have insights into the needs and preferences of their loved ones. Their participation helps us tailor care plans not only to the medical requirements of the patient but also consider their personal preferences and lifestyle choices.


What happens if a patient requires emergency medical attention while enrolled in Pinnacle Plus+?

If there is a need for immediate medical attention during the time our nurse is visiting a patient, they will make the call for an ambulance and wait with the patient until the paramedics arrive and take over.


How often are care plans reviewed and updated to reflect changes in a patient’s condition?

Care plans are reviewed daily to adapt to each patient's needs, enabling prompt responses to changes in their condition. Ongoing communication among the care team ensures everyone is informed about updates.


How can patients and families to provide feedback on Pinnacle Plus+?
